What Is GUIDE?
GUIDE is a new Medicare program that supports people living with dementia and the caregivers who care for them. Through GUIDE, qualifying seniors receive coordinated dementia care, caregiver support and access to respite services — at no cost.

Approximately $2,500 per year (about 76 hours) of respite care at no cost, giving caregivers time to rest and recharge.

How It Works
Getting started with GUIDE is simple:
- Contact Us – Call or email to let us know you’re interested in the GUIDE program.
- Eligibility Check – We review your information to confirm eligibility.
- Care Assessment – Our team completes an assessment and creates a personalized Care Plan.
- Medicare Approval – Approval typically takes 2–3 weeks.
How to Check Eligibility Using Your Medicare Card
Checking eligibility for the GUIDE program starts with your Medicare card. This card includes the information needed to verify coverage.
What to Look For on Your Card
- Medicare Number – This is the unique number printed on the front of your card. It is not your Social Security number.
- Part A and Part B Coverage – GUIDE is available to individuals enrolled in Medicare Part A and Part B.
Your Medicare number is typically shown near your name on the card and may include both letters and numbers.
